Possible Cause

Camera movement during still image capture. The lens is dirty.

Photographer was standing in front of the camera when pressing the shutter button (self-timer shots).

Flash is off even when the lighting condition is poor. As exposure time is prolonged, camera movement will cause the image to be off focus.

The subject is too near.

Rectifications

Press the shutter button down steadily.

Use a lens blower (sold in the market) to rid the lens of dust. Then wipe with lens cleaning paper.

Do not stand in front of the camera. Press the shutter button while looking through the viewfinder.

Either use a tripod stand, or use the flash.

Shoot at a distance of more than 1.2m.
