The following charges apply as described:
A.Non-Recurring Charges
1.Contracted/mandated conversion charge for existing service conversions
2.Contracted/mandated installation charge new installations
3.Add or change individual numbers on subsequent orders
4.Manual Service Order Charges, as applicable.
5.Subsequent activity charges, as applicable.
6.Disconnect charges may apply.
7.Contracted /mandated/tariff charges for other associated/ancillary, as applicable.
B.Recurring Charges
1.Contracted/mandated recurring port(s) charge for specific services
2.Contracted/mandated recurring loop(s) charge for specific services
3.Contracted/mandated recurring feature charges, as applicable
4.Contracted/mandated interoffice facility and mileage charges, as applicable
5.Contracted /mandated/tariff charges for other associated/ancillary services, as applicable
6.Recurring charges subject to one month minimum billing
C.Usage Charges
Depending on the type of call, DS0 Wholesale Local Platform Service local switching rates will be charged to the CLEC. The local switching rate is the same for voice and data usage. There are six usage rate elements and the type of call determines what elements will be charged. The six usage rate elements are:

Per minute of use (MOU)
Per mile per minute per minute of use
Tandem Switching
