•All Polycom SoundStation™ conference phones are eligible for this rebate. Polycom SoundStation™ accessories, such as the extended microphones, and the VoiceStation® are not included.

•Rebate valid on eligible MAX purchases made after August 15, 2007.

•Invoice showing the purchase of an eligible MAX conference phone and Certificate of Destruction must accompany the Claim Form.

•Each Polycom unit is good for one $50 rebate toward any one MAX phone, limit one trade in per
