In order to show adequate proof of destruction of equipment, you must submit photo(s) of the destroyed product (crushed, hammered, etc.) clearly showing the product’s serial number.
I, the undersigned, hereby certify that I have taken the product listed below out of service and destroyed it. I understand that only by destroying the equipment am I qualified to receive discounts towards the purchase of eligible products. I understand that I must either send the product back to ClearOne as proof of destruction OR include a photo of the destroyed product with product’s serial number clearly visible.
