The following forms are required for the ordering of these products: LSR, EU, PS (ISDN BRI only) and the CLEC Ordering Document. Forms are located on the Interconnection Web Site:

Required Fields by Form

All LSR and EU required fields should be populated as required for ordering (Refer to the BellSouth Ordering Guide for CLECs). The next four sections detail the critical fields and entries on the LSR and EU forms and the instructions for the service request (proprietary) form.

Critical Fields and Entries for DS0 Wholesale Local Platform Service - 2-Wire

ISDN/BRI

The REQTYP for ordering the ISDN BRI or ISDN PRI is “MB” (B is 2nd character and means firm order).

The valid Account Activities for 2-Wire ISDN/BRI are “N”, “C”, “D”, or “V.”
