to the FSSA

Service Center.

For the most efficient processing, each piece of required documentation should be submitted on an individual page. For example, if you are providing a photocopy of a driver’s license and Social Security Card, you should copy each document on a separate page rather than copying both items on the same page.

Once an application and/or supporting documents are submitted by mail or FAX to the Service Center, the information should not be resubmitted. A document that is not yet available for processing is not lost; resubmission will slow the system down, creating multiple copies of the same document.

Remember: Each Document Coversheet includes a case-specific barcode that is used to attach the documents to the electronic case file. Do not copy or use a Document Coversheet for client(s) who are not associated with that case.

If submitting an application and supporting documents by mail, be sure to remove all paper clips and staples.
