1. SerialNumber Enter the serial number assigned by the E911 Customer
2. DateReceived Enter the date the Inquiry was received
3. NPA / Telephone
Number Enter the NPAand telephone number being referred on
the Inquiry
4. 911Call Date Enter the date of the 911call
5. Actiontaken Enter the action taken to handle the inquiry
