2Provide the following information along with the equipment you're returning:
•Your company name, address, and the name of the person whom we can contact in case we have any questions.
•The same Purchase Order Number you gave us when you requested your RMA number.
•A description of the equipment, as well as the number of units you're returning to us. Don't forget to include the model and part number of each unit.
•Shipping address so that we can return your equipment to you.
3So we can determine why you're sending the equipment to us, let us know which of the following applies to you:
•The equipment needs an ECO/ECN upgrade.
•The equipment is defective.
Please tell us what you observed just before the equipment malfunctioned. Be as detailed in your description as possible.
•If there's another reason for returning the equipment, please let us know what it is so we can help you.
4Pack the equipment in a shipping carton.
