SIX-MONTH PREVENTIVE MAINTENANCE RECORD

Photocopy this form and use it to record the preventive maintenance performed on the INCLINE TRAINER. Each copy of the form can be used for six months (26 weeks). When maintenance is performed, write the date in the ap- propriate spaces. Make sure to perform each maintenance procedure as described on pages 27 and 28.
